What do I bring with me to my appointment?
Make sure to come in 30 minutes prior appointment time. Driver's License/ Photo ID, Insurance Card(s), and all current Medications. Including the following:

What should you expect from us?
To be treated with respect, dignity and be informed of his/ her car needs to make appropriate decisions. Help plan his/her care and make changes to it. Expect that teaching materials will be provided in a manner he/she can understand To receive care without discrimination due to race, religion, age, sex, disability or ethnic origin.
Do you have same day appointments?
Generally a provider is available to see you the same day. Please call ahead of time and we will do our best to accommodate your needs.
What about blood work?
If the Providers recommend having any blood related lab work performed, please do it at least one week before your appointment. This allows time for the results to be available and ensures you are able to discuss the results, whether normal or abnormal, with the Doctors during your scheduled visit. Generally, we receive blood related lab results within 1-2 business days, but it could take up to 7 business days for Vitamin D and testosterone level tests.
What you need to do before coming to your appointment?
Make sure your insurance card is activated and effective. Make sure your insurance card is set to Dr. Jose F. Polanco as your primary. Inform the staff of any changes in insurance, employment, demographic, and relationships with other care/service givers. Notify the medical practice of any change in his/her health status
I am switching providers and my new provider needs records or I am becoming a new patient to Dr. Polanco and need to transfer over my records.
If you are becoming a new patient we will need you to sign the authorization to release records with your signature so we can fax it over to your previous primary and receive your records. If you are going to a different primary physician you will need to sign the authorization to release records at your new primary's office. Then we will release your records to your new primary.
I want to request my medical records.
If you are asking for a copy of your records we will charge you $1 for the first 25 pages, and 25 cents after that per page. We do not send medical records via email since our email is not encrypted.
What is your appointment cancellation policy?
If you must cancel your appointment, we'll be happy to reschedule at a more convenient time. We ask you to give us 24 hours advance notice in order to provide an opportunity for another patient to be seen
Do you have financial assistance plans?
Our practice is pleased to a financial assistance to patients with limited resources and inadequate medical insurance coverage. Please asked our receptionist for a payment plan form.
What should I do if your office is closed and I need medical care?
If you are experiencing what you believe is a medical emergency, please call 9-1-1 or go to the closest hospital Emergency Department. To page the on-call physician after-hours for urgent matters, please call our office to be connected to the answering service
